Board of Directors Meeting Monitoring Form

In a continual effort to improve the Policy Governance process, a scheduled Board member
completes this form at the conclusion of each Board of Directors meeting. The member evaluates
the Board as a whole and not individual members.

and UNS indicates Unsatisfactory.
